  • The guidelines for the best solution for each needle-free device are not always consistent, so you should know the solution specified by the facility agreement, how the solution works and how to properly apply it to the selected device .

    Wiping method: not just swipe
The wiping method is an active scrubbing method in which the healthcare provider uses a fabric pad (preparation pad) soaked in alcohol and painted on the surface for cleaning. This method is most suitable for multiple quick connector accesses (such as flushing) and multiple administrations. Before entering, use a sterile pre-packaged preparation pad on the needleless connector.

Friction and time are critical to the success of this method. If strong force is applied for at least 15 seconds and allowed to dry passively, alcohol can effectively reduce microbial contamination on the needleless connector. However, most clinicians admit that they do not follow the 15-second recommendation.

The example protocol for this method is:

    Perform hand hygiene.
    Put on clean gloves.
Tear off the sterile wipes or prepare the top of the pad packaging to expose the edges of the wipes. Pull the rag out of the box.
Grab I.V.'s pinless connector device and place it in your inconspicuous hand.
Wipe the thread and septum of the needleless connector with the wipe with your dominant hand, and make sure that the gloved hand touches only one side of the wipe.
Make sure to wipe the wipes on the connector threads (just like squeezing juice) with a few clockwise-counterclockwise rotations. Use the pressure (friction) on the top of the connector to scrub the septum, making sure to clean all gaps.
Twist the wiper on the thread and wipe the septum alternately for at least 15 seconds, covering each area for a few seconds at a time.
Put the needleless connector in an inconspicuous hand and let it air dry before using a sterile syringe or a male intravenous injection. Pipe joints.
Every time you access the needleless connector, follow the steps above to perform a new 15-second scrub.
Method of disinfecting cap: clean and protect
The disinfectant cap is filled with disinfectant, which can clean the needle-free connector before use and protect it from contact pollution between uses. You can screw the cap onto the needleless connector and leave it for up to 96 hours; after putting the protective cap on the needleless connector for at least 5 minutes, the protection begins.

The passive protection provided by the sterilization cap is ideal for intermittent use of central venous access equipment, such as antibiotics every 6 hours. You just need to remove and discard the cap, and you can use the needleless connector without further wiping. The use of a cap on the auxiliary line kit can reduce contamination between two administrations.

This is an example protocol for using disposable disinfection caps:

    Perform hand hygiene.
    Put on clean gloves.
If so, remove the old disinfection cover.
Peel off the protective cover and open the new sterile cover package. Keep the lid in the packaging to prevent touch contamination.
Hold the needleless connector with your non-dominant hand.
Put the sterilization cap on the needleless connector with your usual hand and rotate it clockwise under pressure (just like installing a syringe).
After the time recommended by the manufacturer, remove the cover to use the needleless connector. To remove the sterilization cap, grab the needleless connector with your other hand. Use your dominant hand to turn the sterilization cap counterclockwise to remove it. Discard the lid.
If contact contamination occurs or there is debris such as blood, please wipe the connector with a disinfectant wipe before touching the connector (as described in the wiping method) and before closing the disinfection cap.
Leave the sterilization cap in place until the next visit so that the needleless connector remains clean and protected.
After accessing the needleless connector, close the new disposable cap.

Sterilization of needleless connectors is not a new concept, but it rarely attracts attention. The most important tool for the prevention of central line related blood infections (CLABSI) is still the caregiver, so in the past 8 years, the CDC has always made education a top recommendation
